Compass Concierge Nursing offers private, professional nursing oversight for independent adults who prioritize consistency and proactive care. We support aging in place while providing families with assurances of clinical expertise and professionalism.
We operate on a private-pay, retainer-based model, allowing care to be tailored to each client’s unique circumstances without the constraints of traditional home health or insurance-based services.
Situational clinical support outside of standard service arrangements is offered on an individual basis following consultation.

Foundational
Designed for adults who benefit from periodic clinical oversight and proactive coordination.
For many clients, the greatest value is the steady, informed presence that keeps chronic conditions from quietly advancing and ensures families are prepared and informed at every step. Foundational provides that presence on a reliable monthly cadence.
Includes:
Monthly in-home wellness assessment
Monthly phone/virtual check in
Medication reconciliation, management, & education
Chronic disease management
Compass portal access
Quarterly care plan
Starting at $1,000/month
Active
Appropriate for individuals with evolving medical needs, recent transitions in care, or multiple providers requiring active coordination.
Active provides twice-monthly in-home assessments, appointment accompaniment, and provider coordination so that changes are caught early, families stay informed, and nothing falls through the cracks between appointments.
Includes foundational plus:
Bi-weekly in-home wellness assessment
Bi-weekly phone/virtual check in
Provider coordination
Appointment accompaniment
Monthly care plan
Starting at $2,000/month
Executive
Intended for clients and families seeking a comprehensive, high-touch nursing partnership.
At this level, physical presence and access expand to match the most complex clinical pictures.
Executive engagements are highly customized for both the client and family.
Includes active plus:
Weekly in-home wellness assessment
24/7 priority access to RN
Complex care coordination
Care transition management (hospital discharge, post-op care)
Starting at $4,000/month
We understand that in healthcare, one size does not fit all. Our service tiers are designed to illustrate a framework of services we provide. Final scope and frequency of services are determined following initial consultation. Your investment will be clearly defined before any commitment is made. Our services may be eligible for reimbursement through HSA/FSA. Clients are encouraged to consult their tax advisor or plan administrator to confirm eligibility.
Situational Clinical Support
In certain circumstances, we provide case-specific, clinical involvement outside of standard service arrangements. When appropriate, this may include nursing presence during:
travel
hospitalizations
appointments
transitional periods, i.e., hospital discharge and surgical recovery.
